Direct uploader 403 error on files bigger than 10mb

Recently i started getting 403 error on any files bigger than 10mb.  The upload time for a 13mb file takes less than a minute.  All files under 10mb uploads fine.  Previously, i was able to upload files that were bigger than 200mb.  ACL is properly set to the folder and crossdomain.xml file.  The upload progress bar reaches 100% and fires off confirmuploadJSfunc. I was able to use the S3 Firefox organizer to upload a 250mb file. Have you run into this problem before? Is there anything else i need to check?

i checked the webconfig for execution limits as well:
<httpRuntime executionTimeout="1800" maxRequestLength="1048576"...

UPDATE: i use the DirectAmazonUploader version 1.0.3400.0 and does not return any errors but version 1.0.343.0 returns a 403
